Erica Peralta, LMSW

Erica Peralta, LMSW, earned her Master of Social Work from Lehman College and is an alumna of the Montefiore Medical Center Palliative Care Social Work Fellowship. She currently serves as a social worker with the Adult Palliative Care Consult Service at New York Presbyterian Hospital at Columbia University, where she focuses on supporting patients and families receiving hospice care within the Palliative Care Unit. Erica is an active member of the hospital Bereavement Committee. She has a special interest in research related to the provision of palliative care in Latinx communities and is dedicated to improving the engagement of young adults in advance care planning and end-of-life care.
